What We Do

Impact Health is a local program that is part of a national movement to establish care coordination networks for underserved communities. Within the Impact Health framework, our goal is to foster a sustainable network of Community Health Workers and trained frontline public health professionals who possess a deep understanding of and strong ties to the local community. This program collaborates with agencies that engage Community Health Workers to pinpoint vulnerable individuals and facilitate their access to medical, behavioral, and social services aimed at eliminating obstacles to health and wellness. The program's initial phase is dedicated to addressing healthcare disparities among individuals most susceptible to mental, neurological, and behavioral health challenges.
